Our Purpose

We are dedicated to building authentic community partnerships, fostering trust and accountability, and addressing health disparities—especially for those most affected by kidney disease.
Our approach is grounded in culturally responsive, trauma-informed care, ensuring every community member feels valued.

We embed community voices in our decisions, collaborating with local organizations to promote health equity.
Our work prioritizes historically marginalized populations—Black, Hispanic, Asian, Native, and Indigenous communities—through targeted, culturally relevant programs.

Understanding Community Needs

Each year, we conduct community needs assessments to better understand the diverse identities, priorities, and challenges of those we serve. Led by our Equity and Partnerships Director, this process ensures that patient and community voices guide our work.

By gathering input, we can:

  • Make informed decisions and allocate resources effectively

  • Design targeted services that address pressing issues

  • Build trust through culturally responsive care

  • Measure progress and stay adaptable as needs evolve

  • Strengthen collaboration by leveraging local assets
