Care Manager
Responsibilities
The Care Manager shall use a collaborative practice model focusing on outcomes to enhance the quality of patient care through effective and efficient management and monitoring of patient care programs. This includes comprehensive assessments, planning care, directing and evaluating clinical results of patients, education of patients, as well as services and programs that enhance patient care.
Requirements
The Care Manager must be a RN w/ a license in WA State. Must have previous dialysis experience. Requires training and experience w/ Microsoft Office products and the ability to demonstrate use of appropriate software after training. Experience as a charge nurse preferred. Excellent communication skills are required.
Shift
Day shift, various shifts and hours, Mon-Sat. Active WA State RN license required. One year clinical experience with patient teaching experience. Wa. St. drivers lic. & proof of insurance required.
