Patient Rehabilitation Scholarships
Since 1984, the patient rehabilitation scholarship program has played an important role in our mission to foster the independence of people living with kidney disease.
Patient rehabilitation scholarship application deadline: March 28, 2012
Applicants should have a clear employment goal and be able to demonstrate financial need. The maximum yearly award is $3,000. Scholarship winners may apply each year until they have used a total of $6,000 in scholarship funds.
To be eligible, you must be:
- A current Northwest Kidney Centers dialysis patient or past dialysis patient who has received a transplant within the last five years.
- A Washington resident, 18 years or older.
- Planning to study at an accredited school or training program within Washington state.

Please download the application and instructions. Note that the application has multiple parts, so start early. You will be asked to submit:
- The completed application, which includes pages to be filled out by both your nephrologist and your Northwest Kidney Centers social worker or transplant program social worker.
- A copy of your completed Free Application for Federal Student Aid form.
Turn in the completed application and copy of your FAFSA to:
